Jenks Dance Academy POLICIES & PROCEDURES
Enrollment Fee
$20 per student enrolled; $50 maximum per family
Tuition
Payments are due in monthly installments or by the semester. Tuition is due by the first of every month September thru April. You may pay by check (payable to JDA with child’s name in the Memo section), cash or credit card. Please put your payment in the tuition box located at the front desk or you may drop it in the locked mailbox, just north of the studio building. If mailing, please send to JDA at 9416 S. Elwood, Jenks, OK 74037.
Late Tuition Fees
A $10 Late Fee will be charged to your account if your monthly tuition is not received by the 10th of the month with no exceptions. The late fee must be paid by the next month’s tuition.
Returned Checks
A $25 fee will be charged to your account and must be paid with the new payment and made by the next month’s tuition.
Withdrawals
One month notice from the first of the month is required to discontinue any classes and must be made by the 10th of the month or you will be responsible for the month’s tuition. A Withdrawal Form must be signed and turned in for you to discontinue class. No withdrawals will be accepted after March 1st, 2008.
Change of Class
A Request for Class Change Form must be completed and signed by you and your instructor to change class after enrollment.
Class Attendance & Studio Closings
No tuition deductions or refunds for classes missed due to weather, illness or holidays. JDA follows the Jenks Public Schools Calendar for closings and can be seen at www.jenksps.org Make-up classes are encouraged. Please see your instructor.
Parent Viewing Days
Parent’s are invited to come in and watch the class the following weeks: October 6-11, December 15-20 and March 9-14.
Annual Recitals
Jenks Dance Academy’s Annual Recitals will be held Saturday, May 2, 2009. (*Baby Ballet and Tumbling Tots Recitals will be Saturday, April 25th.) A Non-Refundable Recital Fee of $25 will be due with March tuition. There is no admission fee to the recital. Recital costumes will cost between $35-$55. A deposit of $25 for costumes will be due November 1st and the balance due by January 30th. Costume fees are non-refundable after January 1, 2009.
